Gynecological Disorders and Ayurveda



AMENORRHOEA    
* Physiological causes- Pregnancy, Lactation and Menopause
* Psychological cause- Onset after a period of stress e.g. school examination, going to boarding school. Self limiting; cessation of menstruation in otherwise healthy girls, menses start spontaneously after a few months. Anorexia nervosa may be associated with amenorrhoea.
* Gynecological causes- Congenital maldevelopments of sexual organs such as: Imperforate hymen, Hypoplesia of uterus, Agenesis of ovaries, Chromosomal defects(XO): Turner Syndrome. Oprative removal of uterus; may have been undertaken for surgery. Infection of endometrium, especially endometrial tuberculosis.
* Medical causes- In severe iron deficiency anaemia, Amenorrhoea occurs as a physiological response to conserve iron. Suprasellar tumours like craniopharyngioma causes amenorrhoea. Congenital hypothalamic defects cause failure or delay in sexual development with consequent amenorrhoea. Pituitary infarction (Sheehan syndrome)cause severe postpartum hemorrhage. Persistant  oligomenorrhoea is one of the features of  multiple deficiencies of pitutory trophic harmones.
* Drugs- Cyclophosphamide, Heloperidol, Imipramine, Clordiazepoxide, Methyldopa, Oral contraceptive.

Useful Ayurvedic formulations
1. Decoction of seeds of Gajar ( Carrot ) 20 ml twice a day.
2. Powder of Krishan til 10 gm twice a day with Gur ( Jaggery )
3. Nasht Pushpantak Ras 500 mg twice a day with Warm Water.
4. Rajah Pravartini Vati 500 mg thrice a day with Warm Water or methi decoction or Dasmool kwath.
5. Boladi Vati 500 mg thrice a day with water.
6. Hingwadi Vati 500 mg twice a day with warm Water.


DYSMENORRHOEA ( Katsarava )
There are two types: 
(1) Spasmodic- 
 - Pain begins with flow or only just before it.
 - It is just aching in character often with griping or colicky exacerbation
- Pain is felt in the middle above the symphysis pubis and passing down the anterior surface of the thighs. It is associated with prostration, pallor, headache and vomiting. 
 - It continues for 6 to 12 hours till menstrual flow is well established.
 Causes;
 - Uterine
 - Congenital malformation
 - Ovarian dysfunction
 - Psychogenic
( 2 ) Secondary dysmenorrhoea
- This type of dysmenorrhoea is due to congestion in the pelvis.
- Usually found in the elderly patients beyond 30 years.
- Patients get dull ache in lower abdomen. Pain starts 3 to 5 days before onset of menstruation and usually relieved by onset of menstrual flow.
Causes
Extrauterine: Endometriosis, Pelvic inflammatory disease, Pelvic congestion syndrome, Adhesion's, Structural abnormalities of the genital tract.
Intramural: Adenomyosis, Fibroids/ leiomyostoma
Intrauterine: Submucous fibroids(fibroids projecting into the uterine cavity), Polyps, Intrauterine device, Cervical  stenosis, Infection.

Useful single ayurvedic formulations
1. Kasisa Bhasma 250 mg twice a day with Triphala kwatha.
2. Carpasa kwatha ( Gossypium Herbaceum / Carpus luteum ) 30 ml twice a day with Water.
3. Kumari Ghansara ( Aloe barbadensis ) 500 mg twice a day with Hot water.

Useful compound ayurvedic formulations
1. Saptsar Kasaya 30 ml twice a day with Water.
2. Rajahpravartini Vati 250 mg twice a day with Tila kwatha or kulattha kwatha.
3. Nastpuspantak rasa 250 mg twice a day with  Gud mixed tila kwatha.
4. Phalkalyanka Ghrita 10 gm twice a day with Luke warm milk.
5. Kumaryasava 20 ml twice a day after meal with equal quantity of water.
6. Jirakadyarista 20 ml twice a day after meal with equal quantity of water.

Regimen

1. Barley,  milk, meat juice, oil and long piper is recommended.
2. Manda, Diets and life style causing aggravation of Vata dosa is restricted.

Life style
Unction, sudation and therapeutic enema ( vasti )  is useful.

LEUCORRHOEA ( Swetpradar )

Useful single ayurvedic formulations
1. Tanduliyaka swaras ( Amaranthus tricolour ) 20 ml twice a day with honey.
2. Rohitaka mula ( Tecomella undulata ) Curna 3 gm twice a day with rice water.
3. Lodhra ( Symplocos Racemosa ) twak 3 gm twice a day with water.
4. Vanga Bhasma 125 mg twice a day with honey.

Useful compound ayurvedic formulations
1. Chandrakala Rasa 250 mg twice a day with water.
2. Drakshadi Churna 3 gm twice a day with honey.
3. Pushyanug Churna 3 gm twice a day with  tandulodaka.
4. Patrangasava 20 ml twice a day after meal with equal quantity of water.
5. Ashokarishta 20 ml twice a day after meal with equal quantity of water.

Regimen

1. Wheat, barley, old rice, masura, chickpea, bathua, chaulai, spinach, gourd
( lauki ), papaya, apple, pomegranate, grapes, banana, amalaki, dry dates, ginger (adrak ), coriander, cow's milk, goat's milk and buffalo's fresh ghee  is recommended.
2. Black gram ( urad ), onion, garlic, brinjal, alcohol, pickle, vinegar, sour curd, sour substance, jiggery and spicy food  is restricted.

Life style
Day sleeping, night awakening, excessive exercise, fasting, eating before indigestion of previous food, walking in sunlight, sadness and excessive sexual indulgence should be avoided.

DYSFUNCTIONAL UTERINE BLEEDING ( Raktapradara )

Useful single ayurvedic formulations
1. Udumbar swarasa ( Ficus hispida ) 10 ml twice a day with honey.
2. Ashok valkala ksirapaka ( Saraka indica ) 30 ml twice a day.
3. Amla churna twice a day 3 gm with honey.
4. Dhataki Churna 3 gm twice a day with honey.

Useful compound ayurvedic formulations
1. Pradarantaka lauha vati 500 mg twice a day with durva swarasa.
2. Pradarantaka lauha 500 mg twice a day with kusamula kwath.
3. Asoka ghrta 10 gm twice a day with usnodaka.
4. Patrangasava 20 ml twice a day after meal with equal quantity of water.
5. Ashokarishta 20 ml twice a day after meal with equal quantity of water.
6. Lodhrasava 20 ml twice a day after meal with equal quantity of water.

Regimen

Sali rice, parol, lisora, pomegranate, amalaki, milk, goat's milk, honey and clarified butter ( ghrita ) processed with nyagrodhadi gana is recommended.

PELVIC INFLAMMATORY DISEASE
* Dull pain or tenderness in the stomach or lower abdomen area, or pain in the right upper abdomen. Pain is worse just before onset of menstruation.
* Yellow or green abnormal vaginal discharge that has unusual odor.
* Deep dyspareunia.
* Nausea and vomiting
* Fever

Useful ayurvedic formulations for PID
1. Darviadi kwath 20 ml twice a day with equal honey.
2. Bang Bhasam 250 mg twice a day with honey.
3. Ras Manikya 120 mg twice a day with honey.
4. Pradarantaka lauha 500 mg twice a day with kusamula kwath.
5. Maha Yograj Guggul 500 mg twice a day with milk.
6. Chandraprabha Vati 500 mg twice a day with milk.
7. Arogyavardhini Vati 500 mg thrice a day with water
8. Dashmoolarishta 20 ml twice a day after meal with equal quantity of water.
9. Devdarvadirishta 20 ml twice a day after meal with equal quantity of water.

VAGINITIS ( Yonishotha )

Useful ayurvedic formulations
1. Bang Bhasma 250 mg twice a day with milk.
2. Pushyanug Churna 3 gm twice a day with  tandulodaka.
3. Chandraprabha Vati 500 mg thrice a day with water.
4. Chandanadi Vati 500 mg thrice a day with water.
5. Chandanasava 20 ml twice a day after meal with equal quantity of water.
6. Kutajarishta 20 ml twice a day after meal with equal quantity of water.

Useful local application
Tampons lished with Jatyadi taila should be placed high in vagina.

HABITUAL ABORTION  (Garbhasrava )

Habitual abortion is defined as three or more consecutive pregnancy losses before 20 weeks of gestation.

1.Powder of Shatavari ( Asparagus ) 5 gm twice a day with milk.
2. Mukta Bhasma 250 mg twice a day with milk.
3. Chandrakala Ras 240 mg twice a day with water.
4. Pushyanug Churna 3 gm twice a day with milk.
5. Shatavariyadi Churna 3 gm twice a day with milk.
6. Chyawanprash avleh 10 gm twice a day with milk.
7. Vasavaleha 5 gm twice a day with milk.
8. Ashokarishta 20 ml twice a day after meal with equal quantity of water.
9. Garbhpal Ras 250 mg twice a day with milk till delivery.

Regimen
Avoid sour, pungent food articles like spicy pickle.

Life style
Complete bed rest, foot end raised.
Exercise and excessive sexual indulgence should be avoided.

 It is a natural event that normally occurs in    women age 45 to 55. Menopause is time in
